Important Guidelines for filling up the EPF Forms


All forms should be filled up legibly with correct information duly attested by the employer to ensure faster and early settlement.

SETTLEMENT OF YOUR EMPLOYEES PROVIDENT FUND ACCOUNT; FORM 19

1. NAME OF THE MEMBER : Fill your name legibly in capital letters. The name furnished in Form* 9/5, Claim Form and Bank account should be identical. 
2. FATHER'S NAME : Fill your father or husband name legibly in capital letters claim form and bank account should be identical -- submitted by employer at the time of joining.
3. NAME AND ADDRESS OF COMPANY: Furnish the name of company and full address and correct pin code.
4. Account Number :Please fill up legibly State Code / Office Code / Establishment Code / Extn Code (If any) / Member Account Number. The Account Number allotted to the member shall continue till the member leaves that establishment and the accumulation are settled/transferred.

5. Date of Leaving service : Correct date of leaving should be billed in.
6. Reason for leaving service : From the following please fill the reason applicable to you,
* Retired from Service after attaining 55 years ( OR ) Attained 55 years and resigned from service (OR) Retired on Disability to the work (OR) Under Voluntary Retirement Scheme(OR)Migration to abroad on Permanent settlement (OR) Retrenched from Service (OR) Resigned / Left Service * 
[ * For cases of Resignation / Left Service, the claim should be submitted by the member only after two months from the Date of Leaving service. ]
7. Full Postal address : Fill full postal address should be given clearly in Block letters with correct pincode.
8. Mode of Remittance : It is safer to have the payment through cheque. Account Payee cheque will be sent directly to the Scheduled bank / Coop Bank / Post Office in which Savings bank Account of the member is maintained. If less than Rs.2000 amount will be sent through money order. kindly attach the xerox copy of the first page of your saving bank account pass book.
9. Signature of the member : Member should be sign in the application you should sign the application in the presence of the employer.
10. Attestation by the employer: The authorized official of the company should attest in the claim form with the seal of his designation. If the establishment is under permanent closure get the attest from the following authorized official...
[ Manager of the bank you holding the savings bank account (OR) A gazetted officer (OR) Sub Post Master (OR) MP/MLA (OR) The Magistrate (OR) President of village/panchayat]
11. Advance Stamped Receipt : Wherever payment is sought through account payee, the advance stamped receipt should be signed by the member duly affixing one rupee revenue stamp. The amount need not be filled in which shall be filled in by the RPFC / OIC of the EPFO office.

Important Instructions for filling the Provident Fund Claim Forms


* Furnish the correct information in the relevant columns of the claim forms and use Blue or black ink to fill the claim form Important instructions for filling the claim forms. Fill the columns in BLOCK LETTERS. Please avoid overwriting. Full residential address should be given in the claim.
* Member’s name, Father/Husband name and Date of Birth of the member should correlate with Form 9, 5 details furnished at the time of enrollment.
* The Savings Bank Account Number, Name of the Bank with FULL ADDRESS with MICR Number(if applicable) to be furnished clearly in BLOCK LETTERS. All required enclosures to be attached to the claim and the employer to attest all the attached xerox copies.
* Where the settlement amount is more than Rs.2,000/-, the claimant cannot opt for Money Order as mode of payment. If the withdrawal is preferred by the member, it should be in Claim Form 19. In cases where the claim is preferred by nominees/Legal heirs of the members, it should be in Claim Form 20. 
* All advances/withdrawal for various purpose during the membership in the Fund should be preferred in Form 31 alongwith the enclosures given in the instruction form. In case of membership under Employees Pension Scheme, 1995, 
(i) where the membership is less than 10 years, option should be clearly 
indicated in the Column Number ( ), either for withdrawal benefit or 
for Scheme Certificate in Claim Form 10C.
(ii) in case of drawal of pension, the claim in Form 10D should be 
preferred.
* The claimant should affix his signature/Thumb impression in the certificate portion before the employer. The claimant should also affix his signature in the Advance Stamped receipt column, wherever the Provident Fund/withdrawal payment is more than Rs.5,000/- the signature should be appended after affixing Revenue Stamp.2
* The claim forms should be attested by the authorized signatory of the establishment duly affixing the seal and after due verification of the all the columns filled in the Claim Forms. No joint Savings Bank Account, either or survivor account is permitted. The Savings Bank Account, should be in Claimant’s name. Copy of First page of S.B. account pass book to be enclosed as proof.
* In case of Provident Fund(P.F) beneficiaries are minor, the Savings Bank account should be in Guardian’s Name. MICR Number may be furnished wherever ECS facilities are available for speedy settlement. Form 3A for the current year contribution with due attestation of the employer/Authorised signatory to be invariably enclosed with the Claim Form.
* While joining an establishment, furnish the details of previous employment, if any, with previous Provident Fund account number and Scheme Certificate. Execute Form 2 on joining an establishment/in case of change of nominee with details of self, nominee for Provident Fund and Pension and details of family, so that it is forwarded to Employees’ Provident Fund Organisation by the employer.
* Ensure that Provident Fund is deducted at statutory rate from the total wages i.e. basic, Dearness Allowance(including cash value of food concession), and retaining allowances,  Filled in applications may be submitted through post / courier or in person to the PRO wing of the Regional Office / Sub Regional Office.
* The claims submitted in full complete shape will be processed and settled within 30 days of submission. Claim settlement information is placed in the EPFO website www.epfindia.com and www.epfochennai.tn.nic.in.
* For any further assistance / inquiry, the Public Relation Officer of the Regional Office / Sub Regional Office can be approached on all working days in person or through phone.

How to avail PF Benefits of EPF Members

How to become a PF Member:

If you want to become an EPF member we have to work in a concern which is covered under EPF and MP ACT,1952.
when  a establishment have 20 or more employees are working EPFO will cover for the establishment. If your establishment is not covered and atleast 20 employees are working in that establishment, you can approachEPFO cover it.

How to withdraw money from your PF Account:

If you want to withdraw money from your Pf Account you ahve to either Retire or Resign from the company and apply for settelement of PF in Form-19 and apply for advance for some admissible purposes.   

How to get the pension :

If you have completed a total service of 10 years or more and your are not getting any other EPF Pension and you have attained the age of 50 years or more , then you have to apply for this , use FORM - 10 D at the EPF office where you last worked through your last employer. And also If you want to withdraw pension amount from your account you have to furnish appropriate Post office or Bank address in the application form ( eg: SBI , IOB , ICICI , DFC, BOB,UTI Bank )

Know your PF Balance Through Simple SMS


Recently EPFO India has announced to the EPF members or subcribers or pensioners EPFO has launched a SMS based service for the benefit of all stakeholders. While submitting your claims write your mobile number on top of the form and attach a clear photo copy of the first page of your bank account no. passbook. If the members do it like this they will get money quickly and be informed immediately through sms, After finish your quires a SMS will be sent to your mobile number about the status of your claim.

Some companies allow their employees to contribute more than 12 percentage up to a limit of 24 percent of their basic pay this type of contibution called as VPF ( voluntary provident fund. If you wnat to know the amount of your PF account through an SMS immediately follow the steps ....
First go to the epf official site 
http://www.epfindia.com/MembBal.html 
and select your EPFO office where your PF account is maintained and enter your PF Account number.
The PF account number format is ( State/Establishment code/Account number) ex: TN/67895/98756.
After that enter your Name and Mobile number and agree terms and conditions and the click submit.

How to check and know your provident fund Claim Status ?


we can check the Pf balance through online nowadays it is very easy and simple and know to how to use the facility. 

This facility is very useful for EPF members and subscribers or pensioners who have submitted a claim in any of the Employment Provident fund organisation office all over India. While using this facility one can track the status of a claim so submitted. the only prerequisite is you must know your PF Account number.   

* If you know the EPFO Office from where your claim has to be settled and select the same from the office drop down list.

* On selection of office the mandatory region code and office code will get populated automatically in the respective boxes.
* Enter the establishment code in the third box which can be of maximum seven digits and in case the establishment code has an extension or sub code enter the same here. It can be a digit or letter as the case may be and can be of maximum  three characters in size. Leave this field blank if there is no extension or subcode to the establishment code.
* Enter your account number which can be of maximum seven digits click on submit to get the status.
